This position will work in the Brain Tumor Center, a newly formed joint initiative between Boston Children’s Hospital and Dana Farber Cancer Institute. They will be the main contact for patients and families as they go through their care at both institutions. We are looking for an empathetic, detail-oriented, motivated individual who can help patients and families navigate through these challenging diagnoses and the complex care that follows.
This Patient Navigator will be responsible for:
- Meeting with/calling patients and families to review the implementation of care plans. Referring patients to services frequently used in a clinic and specialized services as needed. Explaining hospital/community-based resources to patients and families, and providing documentation.
- Assessing patients for referrals to resources. Acting as a liaison with internal and community resources. May contact hospital and community resources to ensure access to services.
- May assist patients and families with scheduling multidisciplinary appointments. Reviewing appointment schedule and appointment instructions. Coordinating logistics for plan adherence such as appointment reminders and arranging transportation.
- Assisting patients with applying for insurance. Providing insurance information to care teams to determine eligibility for services.
- Maintaining relationships for accessing community-based resources. Providing outreach and maintaining positive working relationships with providers/administrative staff (including physicians, office staff, nurses, and social services staff). Reporting outcomes of referrals to referral source.
- Documenting patient care and collecting required data. Tracking outcomes and supporting strategic planning processes. Develops and maintains data.
- Attending and participating in care planning meetings to prepare patients for discharge or continuing care.
In order to qualify, you must have:
- High School Diploma/GED required, Bachelor’s degree in Human Services or a related field is preferred
- A minimum of one year of closely-related experience required
- Experience with the clinic population or work with community resources is preferred
- The ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.
- The analytical skills to resolve problems that require the use of basic scientific, mathematical, or technical principles.
- Fluency in Spanish may be preferred.
