Position Summary: The Nurse Practitioner (NP) provides high-quality, patient-centered primary care and infectious disease services for patients of New Hope Wellness Center. The NP works under the supervision and clinical direction of the Medical Director and collaborates with the care team to assess, diagnose, treat, educate, and coordinate follow-up care. This role is responsible for maintaining accurate and timely patient records, supporting efficient clinic operations, and ensuring services are delivered in accordance with applicable clinical, legal, grant, and organizational standards.

Position Responsibilities: (this list is not all-inclusive)
Provide direct patient care, including primary care and infectious disease services.
Perform comprehensive and focused physical exams, assessments, and patient observations.
Record patient medical history, symptoms, assessments, treatment plans, and follow-up needs accurately and in a timely manner.
Develop, implement, and update individualized patient care plans in collaboration with the Medical Director and care team.
Order, administer, and interpret diagnostic tests, laboratory work, and other clinical evaluations within the scope of practice.
Monitor and operate medical instruments and equipment required for patient care.
Diagnose and treat acute and chronic health conditions within the NP scope of practice.
Identify changes in patient health status and adjust treatment plans as clinically appropriate.
Prescribe, administer, and monitor medications and treatments in accordance with licensure, prescriptive authority, and clinic protocols.
Evaluate patient responses to medications, treatments, and care plans, and document outcomes appropriately.
Consult with the Medical Director and other healthcare professionals to coordinate quality patient care.
Educate patients and families on disease prevention, treatment plans, medication adherence, and management of illnesses or injuries.
Support clinic workflows, referrals, scheduling needs, and follow-up appointments to promote continuity of care.
Assist with onboarding, training, and support of new associates as requested.
Participate in quality improvement, compliance, and grant-related documentation activities as required.
Perform other duties as assigned within the scope of the position.
Travel or provide roving coverage to all centers within the assigned market as required.
