Organizational Summary
Alliance Medical Ministry is a 501c3 free and charitable clinic providing comprehensive primary medical care for working uninsured adults. AMM is a Patient Centered Medical Home whose professional medical staff has a mission to reduce health disparities in Wake County. Primary Care Providers collaborate with a team of nurses, medical assistants, behavioral health counselors, social worker, community health workers, registered dietitian, farmer/wellness coordinator, and administrative experts to deliver compassionate healthcare. AMM’s multidisciplinary team aims to address the physical, mental, social, and spiritual issues important for each patient to achieve optimal health.
AMM was founded twenty years ago to increase access to medical care for low-income, uninsured adults, the largest gap in our healthcare system. These individuals do not qualify for Medicaid or Medicare, do not earn enough to be eligible for healthcare subsidies, and cannot afford to purchase health insurance on their own.
Uninsured individuals are at higher risk of going without necessary care even though they are often those with great health needs; about one in six low-income adults are in fair or poor health, and about a third has a diagnosed chronic condition. Currently, we are a medical home to nearly 2,400 low-income adults, with an annual household income at 158% of the Federal Poverty Guidelines. A typical Alliance patient lives in a household of three and supports his or her family on $37,907 a year. To become an Alliance patient, individuals may self-refer or may also be referred from emergency departments, the Health Department, social service organizations or word of mouth. Patients at Alliance pay a flat fee for unlimited number of primary care and in-house specialty care visits. Alliance is ensuring patients are at the forefront by removing financial barriers.
Comprehensive care at AMM focuses on the body, mind and spirit of our patients and incorporates care and services to help our patients improve health and lifestyle outcomes. Key to supporting our patients is to help identify and address barriers to health. AMM provide access to fresh food through our community garden, health education and disease management classes, yoga and other exercise programs, and cooking and gardening classes. AMM patients also have access to mental health services through our pastoral care and counseling program.
In order to achieve our mission, AMM understand the importance of building strong partnerships and collaborations that inspire shared solutions, and as such has and is collaborating with many Wake County organizations including WakeMed, Duke Raleigh Hospital, UNC Rex, Interfaith Food Shuttle, Capital Area Food Network, Alice Aycock Poe Center for Health Education, StepUp Ministry, Dress For Success, NC Works, Catholic Parrish Outreach Food Pantry, You Call this Yoga, Wake County Medical Society Project Access, FIGS, Advocates for Health in Action, YMCA of the Triangle, United Way, and many others to improve the health and lives of our patients.
Position: Primary Care Provider
AMM is seeking a Physician, Physician Assistant (PA), or Nurse Practitioner (NP) provider to work closely with the Medical Director to carry out the mission of Alliance Medical Ministry; provide compassionate, culturally appropriate holistic care; and grow the capacity to serve the eligible uninsured population. The PCP provides preventive and direct medical care, telehealth, contributes to wellness programs, and teams with nurses and medical assistants to lead the delivery of high-quality health services. Physicians may serve as professional supervisor for PA’s or NP’s.

Compensation and Hours:
Part time or Full time. Competitive compensation with IRA, generous paid time-off (PTO), insurance benefits depending on number of hours worked.
Requirements
Roles & Responsibilities:
- Serve as primary care provider for a panel of patients
- Manage chronic diseases, urgent care needs, and hospital or ED follow up care.
- Diagnose and develop treatment plans with the patient
- Oversee prevention, case management, disease management and population health
- Prescribe medications as allowed by federal, state and professional guidelines
- Order/timely follow up labs, x-rays and specialty referrals through uninsured patient programs
- Promote collaboration to both internal and external healthcare team members.
- Assist with training, orientation and clinical support for volunteer providers
- Use Electronic Medical Record
- Cover telephonic call for patient after hours needs in rotation with other PCPs
Education/Training Experience:
- Medical Degree and Board Certification in stated specialty; certified physician assistant; or advance nurse practitioner trained for outpatient primary care
- Current & Unrestricted North Carolina License appropriate to training
- Spanish language fluency preferred
